Have you had a bad experience flying with your pet? If so, please take a moment to file a complaint by downloading and completing the Air Travel Incident Report.
After completing the report, make five copies. Mail the original complaint form to the airline on which you and your pet traveled. Please remember to retain one copy for your records; then mail the remaining copies to:
